Housing Works, a nonprofit founded in 1990 to combat the dual crises of homelessness and AIDS in New York City, has served over 25,000 New Yorkers since its inception. Housing Works provides housing, care management, primary care, job training, preventative services and outreach, legal support, and various other supportive services to over 5,500 people every year. In addition to providing direct services, Housing Works engages in direct action and political advocacy aimed at changing laws and systems that negatively impact the populations we serve.
Position Overview
The Population Health Officer for Quality Initiatives will be based in New York City. The Adult Day Health Program at Housing Works provides nursing, nutrition, mental health and social work support in an outpatient setting to individuals living with HIV and other comorbid conditions (diabetes, hypertension, addiction, mental health issues) with the goal of improving health outcomes and reducing hospitalization and other costly inpatient stays.
The Population Health Officer for Quality Initiatives will work directly with program staff (Social Work, Nursing, IT) to coordinate the various quality management activities already in place, create reports and presentations summarizing quality management activities and outcomes, and will help identify additional opportunities measuring quality outcomes. This position will report to and receive support from the department Vice President, and will also have the support of quality management staff from other departments. Other possible goals may include working with the Marketing Department to incorporate clinical outcomes into promotional marketing materials.
Responsibilities
- Facilitate bimonthly meetings to monitor progress on quality management activities
- Collect and aggregate quality data for monthly program report
- Collaborate with program clinical staff to identify reporting metrics and indicators, as well as develop data collection methods
- Work with Information Technology department to identify gaps in electronic data collection
- Develop presentations that summarize quality activities and outcomes for program staff
- Participate in agency wide monthly quality meetings
Required Skills and Experience
- Proficiency in Microsoft Excel
- Basic knowledge of data analysis and quality improvement
- Ability to develop basic data reports and presentations (Excel / PowerPoint)
- Commitment to public health
Preferred Skills and Experience
- Intermediate statistical analysis skills
- Familiarity with chronic health conditions (HIV, hypertension, diabetes)
- Master’s degree in public health, nursing or social work
Fellowship Logistics
During the fellowship year, all fellows are provided with:
- A pre-tax living stipend of $1100/month
- Housing and utilities

- A professional development grant of $600
- An award of $1500 upon successful completion of the fellowship year
- Travel costs covered to and from placement site, training and retreats
Living and Working in the United States: Fellows are provided with housing and utilities during the fellowship year. Housing arrangements vary based on location within the country. Fellows placed at Housing Works will live with other fellows in a furnished apartment in New York City, New York, an urban center.
